    Query approved!

You may post your question in Mumbai's offline G2G thread as well.

There are a couple of things you can try:
Change your communoication address in SBI passbook with the help of your ICICI statement. If this is possible, nothing better, so check it out. If you are able to get LIC based on your bank statement(ICICI), then that would also serve as an address-proof. Get your PAN card as soon as possible, I believe there are authorized dealers who can do that for you at a basic charge.
Stay away from dubious dealers who look shady, often they will charge you a hefty premium for taking care of registration and most probably will not be authentic.

How about opening an account in govt. Bank based on postpaid mobile bill and company letterhead ? That should do it , right ? I too have same dilemma , with only difference being that I am in Delhi instead of mumbai.
I am looking forward to buy a NS200 , so I went to the nearest dealer and he quoted me the list of documents that would suffice. Postpaid mobile bill and company letterhead are two things that he mentiond would be acceptable.

An attestation from your firm's HR department is valid. It has to contain your PG's address.
This is not just a means of Transportation. Its a vital life component!!

Yeah +1 for the above. An attested copy of the address proof from your firm in its letter head should do.
