Helmet is compulsory everywhere in India, the problem is applying the law.
When the police took stringent measures to make people wear helmets in Pune a few years back even then people would hang the helmet by the handle and wear it the moment they saw cops around.
what we need is more stringent laws and cops who wont cut people loose to make a quick buck.

      3. Cornering mishaps (ran too wide, leaned too much, lost the front end, etc.)
      4. Crashed with oncoming traffic while overtaking,
      5. Obstacles coming in way (cow, dog, pigs, pedestrians, etc.)

      11. 3 way/ 4 way Crossing: Mistake of anticipation (who goes through first, etc.)
      12. Losing grip while braking/ acceleration,
      13. Taking a right hand corner directly, without going circling the chowk/ circle, etc.

Are there any more possible scenarios?
You've covered pretty much everything. I'd like to add a couple more.
      14. Skidding due gravel present on turn.
      15. Ineffective working of Signal lights.
      16. Insufficient light throw of headlight on roads.
      17. Non-working taillights.
      18. Giant speed-breakers.
      19. Panicking due to many factors viz firecrackers, another rider or driver coming in your way suddenly etc,.

Here in Pune, one of the deadliest city in the world to ride, you will often find roads disappearing, dim signals, only one signal light pole present and it's hidden somewhere, huge speed-breakers, so huge even bigger sedans like merc have all wheels on the same speed breaker at the same time, built by military, etc,. Basically govt bodies try to rip you apart in order to keep that 'deadliest' title intact.

+1 to that while I wouldnt call Pune the deadliest (I've seen worse). But the speedbreaker part and part about the traffic signals not being maintained is completely true.

a lot of people in delhi still dont wear helmet, and why only delhi- in other cities/towns etc - i see a lot of people carrying their helmets on rear view mirror, so that if they see some cop, they can immediately wear it and excape fine.

last month i was in delhi and i saw a guy doing this, he was looking quite educated and it was morning time so may be going to office - i asked him on traffic light stop- "boss, if you have an accident, will this helmet (pointing to his helmet kept on RVM) be able to protect your head? does your helmet have some magical effect that even if you don't wear it and are just carrying it along, it will still save you head in case of accident". and he has this puzzled look on his face. just then the signal turned green- and he ran away thinking i might be some traffic cop.

            we need to educate people.
